Adult/Online Design Assessment Surveys

Below is the survey content included for programs with the Adult/Online program design. These are the questions asked of mentees at the beginning, middle, and end of the program, and mentors at the middle and end of the program. All questions are required unless otherwise noted. 

Mentors

Start of Program

For the following questions, please check one response that best describes you. Be honest - this information will be used to help your school and this mentoring program be as effective as possible. There are no right or wrong answers!

  1. I feel like I am an important member of my organization’s community.
  2. I feel supported at my organization.
  3. I am confident that I will achieve the goals that I set for myself.
  4. Once I’ve decided to accomplish something that’s important to me, I keep trying to accomplish it, even if it is harder than I thought. 
    • The scale for questions 1-4 is a 5-point likert scale (1= low to  5 = high)
  5. I have role models who have explained to me how they chose an academic major or career path. 
  6. People important to me have let me know that I am resourceful when it comes to gathering information needed to make career-related decisions.
  7. I have observed people I admire who are resourceful at gathering the information they need to make career-related decisions.
