Program Guidelines:
Please recognize the following as the program guidelines as it pertains to participation in the PROGRAM NAME program.
- Mentees may connect with their mentor via video chat, text messaging, phone, and/or in-person. It is recommended that mentorship pairs meet at least once per month, although more frequent meetings are encouraged.
- To share insights and to track mentorship activity, mentees will go to app.mentorcollective.org and click “Report Conversation” at the end of each conversation with their mentor.
- Understand that the mentor may share updates with the organization to ensure that the mentee has access to the best resources or source of support.
- Mentor Collective provides services intended for educational and informational purposes only. You should not rely on the program as a substitute for, nor does it replace, professional medical advice, diagnosis, or treatment.
- If you have any concerns or questions about your health, you should always consult a physician or other health-care professional. Mentor Collective and its mentors do not assume liability for any losses, injuries, or damages, nor any actions undertaken during or after interacting with a mentor.*

Program Guidelines:
Thank you for completing the matching survey! Please recognize the following as the program guidelines as it pertains to participation in the PROGRAM NAME program.
- Mentors are expected to check-in with their mentee(s) at least once per month via video chat, text messaging, phone, and/or in-person. It is recommended that mentorship pairs meet at least once per month, although more frequent meetings are encouraged.
- Mentors may use the Discussion Guides and other resources provided on app.mentorcollective.org, as needed to supplement the mentorship relationship.
- To share insights and to track mentorship activity, mentors will go to app.mentorcollective.org and click “Report Conversation” at the end of each conversation with their mentee.
- If a mentee is not responsive after two (2) weeks of outreach, the mentor will inform Mentor Collective (via message sent to help@mentorcollective.org).
- Mentors will notify Mentor Collective if they are no longer able to meet the program guidelines, as outlined.
- Mentors may share updates with the organization to ensure that the mentee has access to the best resources or source of support.
- Mentors will treat their mentee(s) as individuals; Mentors will refrain from invoking any biases, sexism, racism, transphobia, xenophobia, homophobia, or any other human rights violations into the mentorship.
- Mentors shall share any serious concerns about their mentee’s physical, emotional, mental, or psychological well-being with the organization and/or help@mentorcollective.org; before doing so, the mentor will seek permission from their mentee.
- Mentors are expected to discuss their intent to participate in the program with their manager. The aim of the discussion is to gain alignment on the time commitment needed as a mentor and gain support from their manager on this individual development initiative.
- Mentors will obey the law and company policies in all activities relating to their participation in this program.
