Student Success Design Assessment Surveys

Below is the survey content included for programs with the Student Success program design. These are the questions asked of mentees and mentors at the beginning, middle, and end of the program. All questions are required unless otherwise noted. 

Mentors

Start of Program 

For the following questions, please check one response that best describes you. Be honest - this information will be used to help your school and this mentoring program be as effective as possible. There are no right or wrong answers!

  1. I feel comfortable at my school.
  2. I feel like I am an important member of my school.
  3. I am supported at my school. 
  4. I am confident that I will achieve the goals that I set for myself. 
  5. I can learn what is being taught in class this year. 
  6. Once I’ve decided to accomplish something that’s important to me, I keep trying to accomplish it, even if it is harder than I thought.
    • The scale for questions 1-6 is a 5-point likert scale (1= low to  5 = high)
  7. In the coming months, how likely is it that you will reach out to professionals in a career or area you're interested in?
  8. In the coming months, how likely is it that you will send emails to professors if you have a question or need clarification?
