Strategic Enrollment Design Assessment Surveys

Below is the survey content included for programs with the Strategic Enrollment program design. These are the questions asked of mentees at the beginning, middle, and end of the program, and mentors at the middle and end of the program. If a start of program survey is desired for mentors, please see/use the student success start of program survey assessment for mentors. All questions are required unless otherwise noted. 

Mentors

Middle of Program 

For the following questions, please check one response that best describes you. Be honest - this information will be used to help your school and this mentoring program be as effective as possible. There are no right or wrong answers!

  1. I feel comfortable at my school.
  2. I feel like I am an important member of my school.
  3. I am supported at my school. 
  4. I am confident that I will achieve the goals that I set for myself. 
  5. I can learn what is being taught in class this year. 
  6. Once I’ve decided to accomplish something that’s important to me, I keep trying to accomplish it, even if it is harder than I thought.
